If you complete this form CHP will store and process your data in accordance with the requirements of its Data Protection Policy and in keeping with the Data Protection Act 1998.

 

You must only use this form for non-urgent repairs.Simply fill out all the fields below and click on the 'submit' button. Your request for a repair will be submitted to the repairs helpdesk who will contact you with a reference number and target completion date.
